How to file a claim

  3. Provide:
    • Your name and contact information
    • Device serial numbers or proof of purchase

  4. Choose your preferred payment method: direct deposit, mailed check, or e-check.
  5. Submit your claim by 11:59 p.m. PST on July 2, 2025.
